Prior to coming in to your dental appointment we ask that you please go over our patient screening questions provided below. Positive responses to any of the questions would likely indicate a deeper discussion with the dentist before proceeding with elective dental treatment.

In addition to answering the questions, we advise patients to check their temperature at home, a reading greater than 100.4 would warrant rescheduling your appointment. Patients will be required to wear a face covering when entering and exiting the building, regardless of symptoms. Arrival Procedures:

As a precaution we are currently not using our waiting room area. Upon arrival, we are asking that you remain in your vehicle and call our new office cell phone number ( 231-492-9097 ) to notify us that you have arrived for your appointment (The office cell phone number is also posted on a bright red sign outside the main entry). We will come to your vehicle, check your temperature and escort you into the building with your mask on. You will be required to wash your hands before being seated in the operatory room.

Additional information:

For the safety of our patients and staff we are trying to keep foot traffic through the office at a minimum, therefore we are asking that family members and drivers remain in their vehicle during the patient's appointment. Doctors and hygienists are available to update caregivers on details/ results following the appointment. Should young children require a parent to accompany them during their appointment, the parent or guardian will be required to follow screening and distancing guidelines.
